Deploy a static site on Cloudflare Pages and collect form submissions with MyFormConnect. Optional: add Cloudflare Turnstile via MyFormConnect spam/CAPTCHA settings.
Prerequisites
- A MyFormConnect account - Sign up here
- A form created in your MyFormConnect dashboard with its UUID copied
- A project connected to Cloudflare Pages (or ready to deploy)

Add a MyFormConnect form to any page you deploy on Cloudflare Pages. Same pattern as other static hosts.

Copy your form UUID
Get your form UUID from the MyFormConnect dashboard.
- Log in to your MyFormConnect dashboard
- Open the form you want to use
- Copy the form UUID (e.g.
550e8400-e29b-41d4-a716-446655440000)

Add the form to your site
Insert the form HTML into your page or template.
- Replace
YOUR_FORM_UUIDwith your form UUID - Commit and push, or upload via the Cloudflare Pages dashboard
- Wait for the Pages deploy to finish

Optional: Cloudflare Turnstile
If you want Turnstile on this form, configure it in MyFormConnect — not only in Pages.
- Follow Spam & CAPTCHAs to enable Turnstile (or other CAPTCHA providers)
- Add your Cloudflare site/secret keys in MyFormConnect domain or form settings as documented there

Test
Confirm submissions reach MyFormConnect.
- Open your Cloudflare Pages URL
- Submit a test entry
- Check MyFormConnect dashboard under Leads
