What is a veterinary intake form?

Veterinary intake records the client (the human) and enough about the animal to prepare the consult: species clues in free text, visit reason, and current medicines or diet the owner remembers. US veterinary practices are generally not HIPAA covered—HIPAA is human health—but owner data still falls under US state privacy laws. UK practices must treat owner details under UK GDPR; RCVS expectations still apply to clinical records of the animal.

The form reuses the medical pack: the ‘patient’ name field is the owner; put the animal’s name and species in the reason or notes fields, or relabel after you adjust field labels.

Owner fields versus animal fields

You are collecting a person’s data to treat an animal. That still means no government ID theatre on a public form.

  • Owner name, email, and mobile for same-day updates from theatre.
  • Animal name, species, and presenting problem in reason/notes (relabel on the form).
  • Current medications or diet in additional notes.
  • Insurance provider name only if you ask—Petplan or nationwide policy numbers can wait for reception.

Do not ask owners for Social Security, National Insurance, or passport scans. Microchip numbers can be added as an extra field; they are not a substitute for identity documents of the human.

How vets collect new-client data

PMS questionnaires, paper, and Facebook messages still dominate first-contact.

Does HIPAA apply to vet clinics?

HIPAA covers human PHI in the US. Owner personal data still needs ordinary privacy care; animal records follow veterinary-board rules.

Can owners upload prior records?

A link to cloud storage is safer than a public dump of another clinic’s PDFs. Prefer bringing records to the visit.

Farm or equine work?

Relabel heavily. This pack is aimed at small-animal first visits.

UK GDPR and pet data?

The owner is the data subject. The animal is not. Still minimise what you keep on a public form backend.
