A new patient form is the welcome packet reduced to what you can safely collect before someone is on your panel. US offices use it to distinguish established versus new billing status; UK private GPs and specialists use it to open a file without pretending to be an NHS GP registration, which still happens through the NHS App or the practice’s clinical system.

It is not a substitute for identity verification at the desk, insurance card scans in a locked drawer, or NHS spine checks. It is a structured email you control, with fields that match how your reception actually phones people back.

What new patients will actually complete

People fill this the same evening they get a text confirmation. If it looks like a hospital admission, they abandon it.

  • Legal name and date of birth matching the insurance card or private-pay invoice.
  • Best callback number including a UK mobile that can receive SMS.
  • Chief complaint or referral reason so the template slot is the right length.
  • Pharmacy or GP name in notes if you will request records later through proper channels—not via this form.
