What is a patient registration form?

Registration creates the administrative identity: legal name, date of birth, contact paths, and sometimes payer hints. US hospitals wrap this into ADT workflows; independent clinics still need the same facts to avoid duplicate medical record numbers. UK private hospitals and clinics register similarly, while NHS GP registration remains a clinical-system event, not a marketing-site form.

Registration without over-collecting

Duplicate MRNs start with nicknames and missing dates of birth. Fraud starts with asking for government IDs on the open web.

  • Legal name as it should appear on invoices
  • Date of birth as the primary matcher
  • Phone and email for reminders under TCPA/PECR rules you already follow
  • Optional insurance or NHS number only when your notice covers it

Do not collect Social Security, National Insurance, or passport scans here. Registration clerks can view cards in person.

Can I add sex or gender fields?

Yes on the form. Follow your clinical and equality policies; do not invent extra sensitive fields without a reason.

What about address?

Add it if you mail results. Many first registrations survive with phone and email only.

NHS numbers on a.com site?

Usually no. If you are not the NHS provider, you rarely need that identifier on a public form.
