A parent-teacher conference form is how families request a sit-down with a teacher during a scheduled window. In US districts that is usually a fall and spring PTC night; in England and Wales it is often branded a parents’ evening. The form’s job is to capture guardian name, pupil, teacher or class, and a preferred slot before anyone prints a timetable.
Paper sheets taped to classroom doors create double-bookings and leave working parents who cannot visit campus at 3 p.m. out of the loop. An HTML form on the school site lets both households in a split-custody arrangement submit independently, then the office reconciles clashes in a dashboard instead of a WhatsApp group.
Ask for scheduling facts, not a full academic history. Teachers already have the gradebook; they need a workable time and who is coming.
