What is a medical consent form?

Medical consent documents that a patient agrees to a named treatment after risks, benefits, and alternatives were explained. US state law and professional-board rules still expect a discussion, not a checkbox marathon. In England, Montgomery principles and GMC guidance likewise treat consent as a process; a web form is evidence of one step, not the whole duty.

The form collects identity, the activity or procedure name you put in the reason field, a typed signature, and a date. It does not render legal advice, capacity assessments, or interpreter certificates.

What a remote consent capture can include

If the person cannot understand the leaflet, no amount of HTML fixes consent.

  • Full name and date of birth matching the chart.
  • Procedure or treatment named in free text so the row is not a blank ‘I agree’.
  • Typed signature plus timestamp via the form backend’s submitted-at field.
  • A link in your page copy to the PDF leaflet they are consenting against.

Do not bolt on passport or SSN uploads ‘for identity’. If you need to confirm identity, do it at the procedure with staff, not via a public file input.
