What is an accessibility request form?

In the US, Title III ADA public accommodations and many employers handling Title I reasonable-accommodation requests need a path that is not ‘email the intern’. In the UK, the Equality Act 2010 reasonable-adjustments duty applies to service providers and employers; the form is how you learn what to change. Neither statute requires a public HTML form to collect a full medical history.

Use request-pack fields: name, email, request type, preferred date (event date), and details of the adjustment. Relabel request type to categories such as ‘BSL / ASL interpreter’, ‘step-free access’, ‘materials in large print’, ‘quiet space’, ‘other’.

Ask what to change, not what is wrong with them

Medical proof, when it is ever needed, belongs in an HR or clinician channel with tighter access—not on a brochure form.

Do not require diagnosis letters, SSN, NI, or passport scans. If an employer later needs limited medical information, use occupational health—not this public form.

US events typically mean ASL; UK events BSL. Put the right language in your options.
