Free HTML Liability Waiver Form Generator

Gyms, studios, and event hosts want a waiver on the booking page. The form should name the activity and timestamps a typed signature, then have a lawyer confirm the surrounding language for your state or for England and Wales.

How to Use This Form Generator

1

Create a Form ID

Export access limited to operations, not the whole marketing team.

2

Paste counsel-approved copy

The generator does not write your waiver text; it captures agreement to the text you publish.

Name the activity field

Pre-fill via HTML value attribute if the page is for one class.

4

Keep records with your insurance file

Dashboard rows are evidence; retention should match your policy.

What is a liability waiver form?

A liability waiver is an attempt to have a participant accept certain risks of an activity. In many US states, waivers can affect ordinary negligence claims if they are clear and conspicuous; they generally cannot excuse gross negligence or wilful misconduct, and consumer statutes vary. In the UK, the Unfair Contract Terms Act and CRA 2015 severely limit attempts to exclude liability for death or personal injury caused by negligence—do not advertise this HTML as a UK ‘get out of negligence’ card.

This form is a collection mechanism: legal name, activity, date, acknowledgement checkbox, typed signature. It is not a substitute for insurance, risk assessments, or counsel-drafted terms.

What the web waiver should capture

A waiver nobody can read on a phone is exhibit A for ‘not conspicuous’.

  • Full legal name as it should appear in any later dispute.
  • Named activity or event so the waiver is not a blank cheque.
  • Date and typed signature plus the backend timestamp.
  • Acknowledgement checkbox tied to on-page risk language you actually published.

Do not collect Social Security, National Insurance, or ID scans ‘to make the waiver stronger’. Identity theatre on a public form creates a data incident without helping enforceability.

How waivers are signed

Clipboards at the door still work. Web waivers catch people who booked last night.

1. HTML on the activity page

A form on your site. Pair with the full legal text above the fold.

2. Waiver SaaS

Smartwaiver-class tools add ID-in-person flows; use them if volume justifies it.

3. Paper at reception

Useful when you must see the person. Store them like any other legal record.

4. Terms buried in a footer

Weak evidence that the participant agreed to this activity.

Waivers on static event sites

Festival and gym brochure sites can post this form without PHP. Keep HTTPS on.

US pages should name the governing state if counsel wants that. UK pages should avoid overclaiming what a waiver can do.

Frequently Asked Questions

Will this stop lawsuits?

No form guarantees that. Insurance, training, and lawful terms matter more than HTML.

Minors?

Use a parental consent flow. A child typing a name on a public form is not a reliable waiver.

eIDAS / ESIGN?

A typed name plus timestamp may count as a simple electronic signature in some contexts. Ask counsel whether you need a qualified e-sign vendor.

HIPAA?

A gym waiver is usually not PHI. If you add health questions, you may create medical data you did not intend to hold.
