Managers should not discover an absence from a Slack emoji. The form lets US employees request PTO and UK employees request annual leave or holiday with start date, end date, and a leave type—then emails the manager who actually owns the rota.
Or one form per team if notification routing is simple.
The hr_leave pack already does.
People request what they just read.
The dashboard is not an approval engine; your email is.
A time off request is an ask against a balance: US paid time off banks, sick days, and personal days; UK statutory and contractual annual leave, plus bank holidays that some contracts include and some do not. The form does not know the balance. It records the dates so a human can check the policy.
Using one form for both audiences means the leave-type select should mention PTO / annual leave in the same option, which this field pack already does.
Dates beat essays. If the rota is the constraint, start and end matter more than a paragraph.
This is not FMLA or UK statutory parental-leave legal advice. Long absences belong on the leave-of-absence generator with HR involved.
BambooHR and Personio have calendars. A 12-person studio often has a Notion page and hope.
Works on a static staff page. Notifications go to the line manager.
Best once balances and public-holiday calendars must be exact.
No audit trail when two people book the same Friday.
Fine until someone is remote in another ZIP or postcode.
Employee handbooks hosted on GitBook or a static /staff URL can include this form at the bottom of the time-off policy.
State whether US federal holidays and UK bank holidays must be requested or are automatic. The form cannot infer that from the date picker.
No. Export and update the tracker you already use.
US copy should say PTO or vacation. UK copy should say annual leave or holiday. The select option includes both phrases.
Put partial days in the reason box, or add a number field on the form.
Use the sick option. Do not demand medical evidence on a public-ish staff HTML page.
