What is a PTO request form?

PTO means paid time off: a US benefits construct where vacation and personal days (and in some policies, sick time) draw from one bucket. A PTO request is the employee asking to debit that bucket on specific dates. It is not unlimited ‘flexible holiday’, and it is not the UK statutory 5.6 weeks of annual leave—though a UK subsidiary can reuse the same HTML if you retitle the page to holiday request.

The form records intent. Accrual math stays in your payroll or HRIS.

What US managers expect on a PTO ask

Blackout dates around inventory or board meetings should live in the handbook above the form, not as a surprise after submit.

  • Name and work email on the company domain.
  • Leave type left on PTO / annual leave or relabelled to PTO only on the form.
  • Start and end dates with a note on whether half-days are allowed.
  • Optional reason — many US cultures do not require a reason for vacation PTO; make it optional.
  • Employee ID if you have duplicate names across states.

If your UK office uses this URL by mistake, add a sentence that annual leave / holiday requests are welcome using the same dates fields. Do not force British staff to say ‘PTO’ in Slack if your policy document says holiday.

Is PTO the same as UK holiday?

No. PTO is a US bundled bank. UK annual leave is a legal minimum plus contract. This page is written for PTO; UK teams should prefer the time-off request generator or relabel.

Does this track accrual?

No.

Can I request PTO in hours?

Add a number field on the form or write ‘4 hours on Friday’ in the reason.

What about company shutdown weeks?

Put them in the handbook. The date picker will still allow those days unless you validate elsewhere.
