Free HTML Consultation Request Form Generator

Consultants and specialist advisors can qualify intro calls on a services page. The form captures the problem and a preferred date so you are not offering a free hour to every tire-kicker who found a Calendly link in the footer.

A same-day no is kinder than a wasted hour.

What is a consultation request form?

A consultation request is a scoped ask for a first meeting: legal, design, fundraising, architecture. US independent consultants and UK practices both use a form to learn the problem before they spend diary time. It is heavier than a generic appointment because the details field is the brief.

It is not a paid engagement. The paid work starts after a proposal. The form exists so the first call is not ‘so… what do you do?’

A brief you can say no to

If you cannot tell whether you are a fit from the details field, the form is too vague.

  • Name, email, phone for a follow-up that is not only email.
  • Request type as general, urgent, follow-up, other—or relabel to service lines.
  • Preferred date as a window, not a promise.
  • Details for budget band, deadline, and what they already tried.

Do not give legal, medical, or financial advice in the auto-reply. The form is logistics.

How intro calls get booked

Open Calendly converts and also fills the week with bad-fit calls. A request form is a filter.

1. HTML on the services page

You reply with a calendar link only if it is a fit.

2. Open self-serve calendar

Use for existing clients. Risky as the only public path.

3. Contact form

Too generic; you will not get budget or deadline.

4. Inbound from Clutch / directories

Still send them here so every lead looks the same.

Static practice websites

A Next.js or Squarespace practice site can host this under ‘work with us’.

If you offer a truly free consult, say the length and what it is not (not a full audit).

Frequently Asked Questions

Appointment vs consultation?

Appointment is a diary slot. Consultation copy signals scoping and advice.

Is the consult free?

That is your policy. Put it on the page, not only in an FAQ.

NDAs before the call?

Send them after this form if you need them. Do not block the form on a wet signature.

Two time zones?

Ask in details. Put your zone on the page (e.g. UK mornings / US ET afternoons).
